The objective of the presentation is to understand the patients' health literacy, as well as their digital literacy, and utilise this knowledge to synthesise a number of educational support tools to achieve optimum clinical outcomes for their CKD journey, utilising the wellbean program as an example.

RN BN PDACN(Renal) MProfEd&Trng. Clinical Team Leader and Treatment Consultant, Partizan Health, VIC Lee is a renal nurse, with a passion for renal education. Following completion of her Post Graduate Diploma of Advanced Clinical Nursing (Renal), she has worked across all areas of Nephrology including Haemodialysis, Peritoneal Dialysis, Renal Transplantation and with Newly diagnosed CKD. She has completed stints in patient and staff education before and after completing a Master of Professional Education and Training. Lee is currently the wellbean patient support program team leader at Partizan Australia, who provide educational support and training for patients with Chronic Kidney Disease who are prescribed Aranesp® for the treatment of Renal Anaemia. |
